Cranford Family Practice Selected to Participate in the TransforMed Project!
The American Academy of Family Physicians has selected Cranford
Family Practice as one of only 36 practices nationwide
chosen to participate in a demonstration project to change the way medicine is
practiced. Among the goals of TransforMed is a shift to more
patient-centered practices and the improvement of the care of chronic illness. Although
we have already integrated several of the processes promoted by TransforMed,
