HOW TO USE THE

PATIENT PORTAL


Easy to set up and use:


  1. ✦If you have not filled out the portal consent form download the Portal Authorization form and drop it off or mail it in to the office.


  1. ✦Once the completed form has been returned to the front office, you will receive an email from

GoToMyClinic.com/cranfordfamilypractice. 


  1. ✦Follow directions in the email to log on, change your user name and password, then you are set-up and ready to go.


  1. ✦If you are having difficulty in using the portal or would like additional info, check out the patient_portal_guide.pdf

 
 

✦Preventive exams including:

  1. •Complete health assessments and physical exams

  2. •Women's health exams including pap smears

  3. •Immunizations

  4. •Detection of colorectal cancer

  5. •Screening for skin cancer

  6. •Testing for prostate cancer
