HOW TO USE THE

PATIENT PORTAL


It is really easy:


  1. ✦If you have not filled out the portal consent form download the Portal Authorization form and mail it in to the office.


  1. ✦Once you have filled out the form, you will receive information from

GoToMyClinic.com/cranfordfamilypractice.  Just log on and change your user name and password and you are ready to go.

 
 

✦Preventive exams including:

  1. •Complete health assessments and physical exams

  2. •Women's health exams including pap smears

  3. •Immunizations

  4. •Detection of colorectal cancer

  5. •Screening for skin cancer

  6. •Testing for prostate cancer

  7. •Evaluation of risk for heart disease

  8. •Evaluation of risk for osteoporosis

✦Care of acute illnesses including (but not limited to):
